Priority Care, a program of the Humboldt IPA, is available to members with  the following health plans
Anthem PPO & HMO Blue Shield HMO	Blue Lake Rancheria Health Plan Self-pay option also available

Enrolled patients will work with a care team to support specific needs and goals. Each care team includes:
Nurse Practitioner Registered Nurses Medical Assistants Marriage Family Therapist/Addiction Specialist	Certified Diabetes Educator Certified Personal Trainers Wellness Coaches

Services Include:
Home, Hospital, Skilled Nursing Facility visits Coordinate care between health care providers Create individualized diet and exercise plans Group exercise classes Provide stress and/or anxiety management techniques	Smoking cessation Streamline labs, appointments, and avoid ER visits Provide information and education about condition(s), medication, and treatment options Accompany patients to medical appointments And much more…
